Level 3 Specialist Support Coordination

High-Level Expertise for Complex Support Needs

At Aus Care Community Services, we understand that some participants face significant barriers that make navigating the NDIS particularly challenging. Our Level 3 Specialist Support Coordination service is designed for individuals with complex needs, high-risk situations, or significant barriers to accessing and maintaining support.

What Is Specialist Support Coordination?

Specialist Support Coordination is tailored for participants who experience:

  • Complex or high-risk situations (e.g. housing instability, family breakdown, or mental health crises)
  • Involvement with multiple service systems (justice, child protection, homelessness services, etc.)
  • Significant barriers to accessing or coordinating support services
  • A need for ongoing specialist support due to behavioural, cognitive, or psychosocial disability

Unlike Levels 1 and 2, which focus on building skills and capacity, Level 3 provides stabilisation, system navigation, and problem resolution at a high level of complexity.

What We Do in Specialist Support Coordination

1. Managing Complex Support Environments

We coordinate and align multiple services across sectors, including:

  • Disability services
  • Health and mental health systems
  • Housing providers and SIL services
  • Legal and justice systems
  • Behaviour intervention teams
  • Emergency services and crisis housing

2. Crisis Planning and Response

We develop personalised crisis plans to prevent service breakdown and reduce risk. If emergencies occur, we act quickly to:

  • Maintain accountability and transparency

5. Building Long-Term Solutions

Our aim is not just to stabilise, but to move you forward. We support you in:

  • Developing goals and adjusting them based on current circumstances
  • Accessing long-term housing support, allied health, community programs, and behavioural support
  • Establishing routines and formal support structures that reduce future risk
