Empowering participants to access the right support, quickly, clearly, and confidently.

At Aus Care Community Services, we make it easy for NDIS participants, families, support coordinators, and referrers to connect with the supports we provide, including Support Coordination, In-Home Support, Social Support, Respite, Community Reintegration, Behaviour Support, and NDIS Housing Support pathways.

Where allied health is required (e.g. NDIS occupational therapy, speech therapy, physiotherapy, behaviour support, or NDIS early childhood intervention), we coordinate with external providers to support integrated, participant-focused outcomes.

What Is NDIS Support Connection?

Support Connection is the first level of NDIS Support Coordination. It’s designed to help participants understand their plan, identify supports, and take the next steps with confidence. This support is short-term and practical, focused on connecting you with the right services and building your ability to manage your plan over time.
