Aus Care Community Services works alongside health professionals, support coordinators, plan managers, hospital teams and families to connect NDIS participants with the support they need across Sydney and NSW.

Who Refers to Us

We receive referrals from:

  • hospital discharge planners and social workers
  • occupational therapists, psychologists and other allied health professionals
  • support coordinators and plan managers
  • GPs and community health teams
  • families, carers and participants themselves

NDIS Support Coordination

Help to understand a plan, connect with providers and coordinate supports, including Specialist Support Coordination for participants with more complex situations. Support Coordination can also be delivered remotely, wherever the participant is in Australia.

In-Home Support & Personal Care

Help with personal care and everyday tasks at home.

Social & Community Access

One-on-one support to join community, social and cultural activities.

Home and Living Supports (1:1 SIL & Respite)

One-on-one Supported Independent Living and short breaks (respite).

How We Work Alongside Referrers

A Care Navigator for Every Participant

Each participant has a Care Navigator as their main contact, who coordinates supports and keeps communication clear.

Primary and Secondary Support Workers

We match each participant with a primary and a secondary support worker, and arrange a meet-and-greet before support starts.

Keeping You Informed

With the participant’s consent, we keep referrers updated on how supports are going.

Registered Provider

Aus Care Community Services is registered with the NDIS Quality and Safeguards Commission, including for Supported Independent Living.

Plan Reassessments Under the New Rules

From 27 August 2026, the NDIA says only a participant, their nominee or their child representative can request a plan reassessment. Support coordinators, allied health professionals and other providers can’t lodge the request on a participant’s behalf.

Evidence from health professionals can help participants show what has changed. The NDIA generally looks for evidence that describes significant, ongoing changes in functional capacity and daily support needs.

Where Support Coordination is funded, our Care Navigators can work with you and the participant to organise information and prepare for conversations with the NDIA.

What We Need in a Referral

To help us respond quickly, please include:

  • the participant’s name and contact details, and their preferred contact person
  • NDIS number and plan management type (NDIA-managed, plan-managed or self-managed), if known
  • the supports needed, and how soon
  • relevant background, such as a recent hospital stay, living situation or risks
  • any reports or plan details the participant agrees to share
  • your name, role and contact details

Please make sure the participant (or their nominee or guardian) has agreed to the referral.

NDIS Hospital to Home Discharge Support

For participants leaving hospital, early contact helps. We can work with discharge teams to coordinate support at home. The treating hospital team decides when a participant is ready for discharge, and the NDIA decides what the NDIS funds.

Who can make a referral?

Health professionals, support coordinators, plan managers, hospital teams, families, carers and participants can all make a referral, with the participant’s agreement.

Does the participant need to have an NDIS plan?

Can you take urgent referrals, such as hospital discharges?

Yes. Please call us on 1300 768 092 to discuss urgent referrals, so we can talk through what’s needed and our availability.

Will you keep me updated after a referral?

Yes. With the participant’s consent, we keep referrers updated on how supports are going.

Can support coordinators request plan reassessments?

According to the NDIA, from 27 August 2026 only the participant, their nominee or child representative can make the request. Providers may still help participants prepare.
