Social & Community Access

Support is one-on-one. Your support worker helps you plan the outing, get there, take part and build the confidence to do more on your own over time.

• Planning outings and activities with you
• Support to travel to and from activities, where transport is funded
• Joining outings and community events, including events hosted by Aus Care Community Services
• Building social and communication skills in everyday settings
• Support to try new activities, volunteer or join community programs

Regular social participation can help with:

• wellbeing and feeling less lonely
• communication and social skills
• independence and confidence
• working towards your NDIS goals
• a sense of belonging in your community
• practising everyday skills, like problem-solving, in real settings

Activities are planned around the goals in your NDIS plan, like making friends, getting more active, learning a new skill or getting out more. We start by listening to what you’d like to do, then plan activities that suit your interests, routines and support needs. As your goals change, we review your activities with you. We match each participant with a primary and a secondary support worker, and arrange a meet-and-greet before support starts.

Common activities include:

• Arts and crafts workshops
• Cooking and nutrition programs
• Music, dance and movement sessions
• Fitness activities, like walking or yoga
• Community BBQs and social meetups
• Movie nights, games and social events
• Day trips to museums, beaches and local attractions
• Time outdoors, like farms, parks and waterfalls
• Cultural and faith events
• Life skills workshops, like budgeting or meal planning
• Supported volunteering and community programs

Every participant has a Care Navigator as their go-to person. They help plan your activities, check in on how things are going, and adjust your support when your goals or needs change.

Aus Care Community Services is an NDIS Quality and Safeguards Commission registered provider.

Our support workers are experienced in disability and mental health support. They follow safety procedures on every outing and plan activities around each participant’s mobility, support needs and comfort.

In your NDIS plan: this is funded under Assistance with Social, Economic and Community Participation, in your Core Supports budget. Some plans also include Capacity Building supports for building social skills.

From 1 October 2026, the Australian Government says budgets for social, civic and community participation supports are being reset progressively as plans are reassessed or renewed. Talk to your Care Navigator about what it may mean for your plan.

What’s changing in the NDIS in 2026 →

• NDIS Support Coordination: help to understand your plan, find providers and bring your supports together.
• In-Home Support & Personal Care: help with personal care and everyday tasks at home, planned around your routine.
• Home and Living Supports (1:1 SIL & Respite): one-on-one support to live more independently at home, plus short breaks (respite).

Social & Community Access often works alongside other supports in a participant’s plan, such as:

• NDIS Support Coordination, to link your activities to your wider plan goals
• Behaviour Support & Allied Health, including strategies for positive interactions and therapy for communication and social confidence
• NDIS Community Reintegration Support, for returning to community life after illness, a hospital stay or a long time at home

Your Care Navigator can help connect these supports and fit them into your plan.

1. Check your plan. Look for Assistance with Social, Economic and Community Participation funding in your Core Supports budget. Your Care Navigator can help.
2. Speak with a Care Navigator. Tell us what you’d like to do, and when.
3. Plan your activities. We’ll set up a service agreement and agree activities that suit your goals.
4. Start your support. Your support worker joins you, and your Care Navigator checks in.
5. Review as things change. We adjust your activities when your goals or needs change.
